go to Page Layout > Print Titles > Header Footer Tab > Custom Header > click on folder icon (to inser path) and insert "/" and then click on icon next to folder icon (for file name).

I think the chart is right. the first value in the series is plotted in left bottom and then go up in order. to get first value in upper corner you need to reverse the order of the series
right click on left axis>Format Axis > Axis Options > Categories in reverse order...
