Use CHOOSE Formula instead of IF

Hi,

Please find attached sample file.
Yellow Highlighted (D3:D25) If formula is Correct
Orange Highlighted (E3:E25) I want to use CHOOSE FORMULA
Can anybody help.

Although you asked for a Choose Formula, because of the different age ranges, I'd suggest you look at Faseeh's formula.
It is much more flexible
