Hui Excel Ninja Staff member Feb 18, 2014 #2 Setup a form with fields as per your sample The form could be linked to a Database or another worksheet where you enter the values and it will update on the form for you Best to start simply laying out the form first

Hui Excel Ninja Staff member Feb 18, 2014 #6 pAvi As with the Saab file it is simply a matter of laying out the cells in the style you want you can either add cell border or add shapes ontop of the worksheet for check boxes etc There is no easy method Start at the top and work your way down You can merge cells and add shading to cells and cell boarders to get various effects Don't be shy on using more columns than you need as you can resize or delete them later if you don't need them
