You will need to setup an area where you have ages down the side and then various columns for Male/Female premiums and then I assume you will have various mulipliers etc for things like smoking, drinking, general health and other factors.

Populate the table with values or formulas

setup an area to collect data Age, Sex, Smoking, Drinking etc

Retrieves the appropriate data using Vlookup, Index/Match etc

Calculate some premiums
