Formula that adds 1 everytime a duplicate name is entered

I have a list of Last names in column B and column C I have the amount of times a client has come in. I want a formula that adds 1 to every time the client comes back. I don't want column C to have the total number but a growing number.

8/1/2016 - Smith - 1
8/2/2016 - King - 1
8/3/2016 - Smith - 2
8/4/2016 - King - 2
8/5/2016 - Smith - 3

In the example above, Smith came in on 8/1/2016 the first time and on 8/3/2016 the second time, and on 8/5 the third time. I want the formula to have an expanding number.

I've attached an example of what I want.

I do realize that my explanation is confusing and any help would be greatly appreciated. Thanks

Your file show Last name in column B, First name in column C, and the Result of Count of name in Column D, which appeared difference in your Post # 1 description.

Thus, "Result of Count of name " D2 formula copy down :

=COUNTIFS(B$2:B2,B2,C$2:C2,C2)
