VLOOKUP Malfunction

I have now just come across a new issue in my Random Drug Testing spreadsheet. I have a VLOOKUP formula that keeps dropping off my last two values. If you count Column C it contains 112 names. When you compare that to the Column D it only lists 110 names. I have spent the last two days on the Net searching all the VLOOKUP, LOOKUP, INDEX/MATCH formulas and have not been able to identify how to correct my VLOOKUP issue. As you will see the formula is working and not displaying any error messages.
Any thoughts is greatly appreciated!! Thanks,

Thank you for such a quick response. Now that the crisis is over, Does VLOOKUP have a limitation on how many rows it can view? Potentially this unit can have 118 patients. Should I modify the formula to accept 118 rows? Average census runs in the eighties, but I will not be the one using this tool on a day to day basis. You know how it is, you create a tool to meet a customer's needs and the first time it hiccups, the creator and the tool are burned at the stake.

Hi:

I have not done anything fancy with your formula, the issue was with your small function and as per my knowledge I don't think there is any limitation on the nof rows for a Vlookup function.
