This is a variation on a cross-tab unpivoting exercise.
The parameter and column field are given by
= INDEX(header,[@Row])
= INDEX(grid,[@Row],[@Col])

where the row and column numbers are
=1 + MOD([@k]-1,6)
= CEILING( [@k], 6 ) / 6
