There is a slightly different (better?) form of LOOKUP that returns the result from a vector rather than the right-most column of an array. Using structured references this would read
= LOOKUP( Achieved, Table1[From], Table1[Slab] )

As both Hui and Bosco observed there is also inconsistency in the way in which you have specified percentages so, as it stood, your formula did not return "Yes" from the correct cell.

As #7, but 'old style'
for K7 =LOOKUP(K5/100,C4:C10,B4:B10)
as You didn't use %-value in K5

My mistake, should read as :

=LOOKUP(K5*1%,C4:E10)
