Need help to create an Excel Table and Chart

I need some help to put the following information into Excel table and auto-calculating the yield and plot a chart.

I manually calculated and input the yield into the Yield column.
As the list increases or in between I need to change the pass/fail data, I will need to manually calculate the yield again.
Is there a better way to do so?

The formula so my problem.
I just need to figure out which chart the best presenting the data.

Why my formula in column "M" not working and it won't show "Failed" when "N.A", " ", or others were keyed into columns J, K, L?
=IF(AND(J3=K3,K3=L3,L3=J3),"Passed","Failed")

and is the description in column "S - Overall Yield %" correct?

The yield in the first picture is correct
In the picture above it should be
100%
50%
66.67%
75%
80%
83%
85.6%

You seem to have the inverse, 1-the above values

The yield in column S is based on the overall result in column M.
Column K and L (input by users) are used to determine the M result.
This is where my formula come in place in column M.
The condition will be if any column (K & L) are left empty, N.A, the M result will indicate Failed, and Overall yield will then auto calculated.
