Hi,

1] I made some changings in your Output Table heading (AA2:AC2), all heading description must be as same as the Source Table heading.

then,

2] In AA3, formula copied across and down :

=IF($A3="","",INDEX($A$3:$V$10,ROWS($1:1),MATCH(AA$2,$A$2:$V$2,0))&IFERROR("/"&INDEX(AA4:AA$10,MATCH(1,INDEX(($A4:$A$10=$A3)*($D4:$D$10=$D3),0),0)),""))
