Index Match Multiple Criteria

I tried using Index Match and was not successful. In this problem I tried to extract data into 2 rows which are in green color.
If one employee works in 2 different states what formula can i use to extract the data. I have all my data in data sheet and a W2 form in W2 sheet.

Expected Result is in Green color

I added employee ids for each employee. I have an employee Krishna Smith with 2 employee ids working in 2 different states. Any ideas.
