How to concatenate a text and a number with leading zeros so that zeros remain intact

Dear All,

I am facing a problem where I need to concatenate one text string and a number with leading zeros to form an ID. I am attaching the excel with a sample and desired output. Could you please help. I am using MS Office 2010.
