how can i export excel dates into outlook calendar

hello all,

in the attached file i would like to have the call on field and the phone no and ph.II fields to be imported to outlook calendar.

i have tried using the import/export feature in outlook. have converted the file into csv but am not sure how this works as the dates get changed when i do this.

kindly help.
