I am able to get the form to update. But, I am having a problem with duplicate. I am using D5, D6 and D7 as unique values criteria's. I am using the CheckNameID to work like and SQL statement.

Secondly, I have a problem with my cell formula's being remove when the Add or Clear button happens.

Thirdly, the unique values are being overwritten in the LesseData sheets.

Again, I am using the example on the contextures site as a guide.
