Formulas and data validation list doesn't copy down when adding new rows to a table

I have a table saved in table format. When I add a new row, the formulas and the data validation list doesn't automatically appear in the newly added rows like it used to. What is causing this, and how can I fix this?

I am unable to post a file because of confidential company data.

with same kind of table
with some same kind of sample data

I ended up creating a new tab and a new table. I think I might had added new columns with these features, then extended the table to include these columns, and maybe that's why it's acting this way.
