Extract Month and Year from a Date cell

Please help. I would like to use MONTH and Year functions and pull from a cell with month day and year. I would also like to combine the functions with text.


Example:


Cell A1: 01/12/2013


Want cell B1 to return BANK RECONCILIATION - JANUARY 2013


I can get either the month or the year to return correctly, but not both.
