Excel VBA User Form output to fill PDF form?

I am trying to write Excel VBA code that will take inputs from a User Form and use those inputs to populate fields in a PDF form. I am looking for the simplest possible way to do this, so if anyone has done it or knows how to do it easily, that would be extremely helpful! Thanks so much!
