Creating Add-Ins for Conditional Average

I have written a formula to calculate conditional average, Now I want to add this formula as my own formula as an excel add-ins.
So if someone else is working on any other worksheet with different set of data, then they will just have to add the Ad-Ins, and whenever they type conditional average, it will start poping up the pre-defined formula, and they will just have to feed the required input only.

Its there in my Excel version too, but unfortunately i am having trouble using the same.
