Chart that automatically updates and displays the last 12 months

I need help with a formula for a dynamic chart I want to create.
The data is sorted like this.

KPI Target
Okt 13 99,34 99,5
Nov 13 98,16 99,5
Dez 13 99,94 99,5
Jan 14 99,93 99,5
Feb 14 99,9 99,5
Mrz 14 99,69 99,5
Apr 14 99,94 99,5
Mai 14 99,73 99,5

I need a formula for the last 12 months. The chart should update automatically (allways showing the data for the last 12 months)

Use the Search Box at the Top Right of this page and lookup Dynamic Chart

I have a different chart with a similar problem. I used this tutorial: http://peltiertech.com/Excel/Charts/DynamicLast12.html

It works if I directly link the data to the chart. But not in my version with the combo box.
