From UserForm, right button on Controls, Additional Controls, and check these:

Microsoft Date and Time Picker Control 6.0 (SP4)

Microsoft MonthView Control 6.0 (SP4)

Microsoft Outlook Date Control


I guess that the second one is what you're looking for.
