very basic user needing help finding what formula to use

Hello, I am a very basic excel user and I need help on what kind of formula to use for a spreadsheet I am needing to create. I am wanting to create a spreadsheet that when I select a persons name from a drop down box, it will return and ID(one I have specified) in one cell, and in another cell it will give me a diagnosis code. All of this information will come from a table that I have created in another sheet. It will look something like this. Thanks.

Using VLOOKUP(): If I have a drop down list of names, I select the name I want say, Sara. Then, when I select Sara, then in one cell it will give me her ID, and in another cell will be her DX code.
