Unique List of Names between two Dates

Is there a formula to get Unique list of names appearing between two dates from a table populated in a range.

I was also working on this at home and tried to do something as in enclosed file.

(Was trying to mimic as per the link: http://www.get-digital-help.com/200...lumn-where-an-adjacent-column-meets-criteria/)

Not sure where and what i was goofing up. Can you please have a look and if possible figure it out. (Just for my learning).
