Return value of number between low and high values

I'm trying to convert a number that is output from a weighted formula into a position between two separate values. The weighted formula takes in several factors and outputs a value between 5 and 71. I need to convert that to a value between 2.75% and 6.25% using a formula.

Another two option (apply to @NARAYANK991 example... without calculate the Scale Factor):
=TREND(B4:B5,B1:B2,E1)
=FORECAST(E1,B4:B5,B1:B2)
