For my office activity, I have some questions (optional type ) and I need these qustioned have to raised for employee in survey format. And employee answer should autometically placed in common path. Kindly assist me is there any software or for the same.

Optional type ans (You can do this in excel by using drop downs).

What do you mean by answer should automatically placed in common path?
