Occasional And Unpredictable User Error Generating Command Bar

This is a cross-post. I'm not usually one to do so, and I hope I'm not committing an egregious sin in the hallowed halls of this shiny new forum, but so far no one in the Mr Excel community has had any advice or suggestions.

The problem is an error code which only periodically affects a small number of users when invoking a macro at startup. Said macro creates a toolbar with several macro shortcut icons in the Add-Ins section of the Ribbon. More details can be found here:
