Need to fetch values from a less symmetrical data

Hi fellows,

I need to fetch values from a less symmetrical data format. I have attached the sample workbook with 02 sheets named "Input" and "Output". Input sheet contains the raw data format which I used to download from an online portal and want an output in excel as mentioned in output sheet from this downloaded format. Thanks in advance for your great help as always.
