Macro to relocate & resize merge cells to another location with text, based on cell value

Hi Guys
The task is to keep this form compact as much as I can. The manager are able to comments in comments box ,but when one cell is filled than those merge cells should relocate to another position with text.
Attached is example, it will make more sense
