Macro: Copy data from sheet 1 over to sheet 2 in different format

Hello All!

I have a form on Sheet 1. This form takes account number, items and quantities, phone number, and a name of a customer. I need a Macro that will take the data from those fields and move it to Sheet 2 which will be a straight horizontal format.


I also need this macro to clear Sheet 1 after moving the data and then when the Macro is run again when new data is put in, to be at the next blank line on Sheet 2 as to not overwrite the first data transfer.
