Lookup and Return Multiple matches across multiple sheets

Dear Sir/ Madam,
please find enclosed herewith an excel workbook which contains four pages: three of them hold sample data that are to be looked up and the fourth sheet holds the table that requires a formula to fill up the cells.

Is it possible to build a formula that lookup and return multiple matches across multiple sheets?
Vide the attachment and clarify
