latest entry data against unique number

Hi All,

hope all is doing well,

i have one raw data excel file where i have unique numbers, i need against unique number what are latest date and counts of particular name, sample file attached of raw and needed results.

thank you. i just need one condition, if possible, when i put number in result sheet so data check & match 1st Sr no and then number and show number latest count and date .
