Inputting Birthdays and Anniversaries using user forms

Hey, I have tabulated all my birthdays and anniversaries in Excel by directly entering the values. Now moving further I wish that they should be entered by a user form and automatically segregated to respective date. is it possible. ???

I have this video but the problem is segregation of data, it adds all the data in the single sheet in the order it is added, I want this to be in a segregated form. Distributed month wise,
