index match with 2 criteria and 2 header columns

I have a table with Year, Month, Company 1 and Company 2 columns. Trying to use INDEX and MATCH to extract Company 1 and Company 2 data based on Year and Month criteria

I am getting #REF! error if I change the month from Jan to Feb.
