index & match for this multiple criteria

hi,

been working on this project on how to get the exact data from another worksheet with multiple criteria.

in worksheet 1 i have the list of areas corresponding the number of customers per week.
in worksheet 2 were the variables that i needed a result. But in the variable of morbidity week was in drop down box. that every time i change week i can get results.

Another option without helper cells

In "Sheet3" B3, copy across and down :

=INDEX(Sheet1!$C$2:$Q$7,MATCH($A3,Sheet1!$A$2:$A$7,0)+(B$2="Alert Threshold"),$B$1)
