how to create a new excel file with columns listed in one file

I have one file which has three columns .
Table Name
Column Key
Column Name

Using these value i need to create the new excel file . Excel file should contain the sheets with table name. Inside each table name sheet, I need to create the first row with the column names.

i have mentioned as sheet name in the sample file . sheet name is nothing but table name as i have mentioned early
