How Replace Multiple Cells Data Replace

I have Data in One Workbook and another workbook i have Old Value & New Value i want to replace old value with new please tell me how to do this

I have this macro but Same work Book Old Value & Old + New values then work please tell me how to do this in different work book then hoe to do this
