Help required for macro

Dear Friends,

I have attach herewith a "Merge Worksheets Code" example file found from this side. A very useful file this is(for me at least), as i don't know the macro well i am asking your help. what i need:
1> Answer Sheet name should be "Output Total" in place of "RDBMerge Sheet"
2> When "Test 2" running, the output must include the Headers at the first row(the Headers should be same in each sheet in my data will be same) & need the Sheet name at last of the each row which is in "Test 1".

but where is the "Sheet name" at the last of each row like "Test1"? please give some of your valuable time for this.

i have done it myself, but not confident... will you please confirm that the code of "Test2" is ok, thanks a lot for help in advance
