Half Completed Userform Please Help

I have this Half Completed Userform

What i want is that when i write a work on the blank bar, this word will be input to the (C) column. Automatically always writing on column (D) FOLDER.

For a clearer visual please look at attachment with file.

I would really appreciate if you guys may help me with this. Thank you guys again, Chandoo

P.S the userform Pop up is on the ADD FOLDER box when clicked
