Get result based on texts in different cells.

Hi, Good Day!!

I am in the preparation of employee list for their health insurance renewal. I would like to get premium based on data in different cells. For Example, I have data such as name , date of birth, gender, class (VIP, A, B,,) etc.

So I want to get same premium for
Male employee with a particular class
Married female employee with particular class
Unmarried Children with particular class , etc.

Good to hear back from you. The first thing I want to suggest you to form a standard condition table; with all the combinations and related premium than it will be easy to figure out the formula for the solution.
