Formula help for consolidation of days to months..

Dear Gurus,

I have data such that on daily basis queries is being entered.
As I am trying to distribute the same as chart , I am in need of a formula that will consolidate / group the days into monthly split up.

I tried the same in PIVOT but when tried to call the dates into column axis,unable to group the same.

I have attached the sample file.

Kindly help.
