Excel-2013-Help on lookup value

Hi Folks,
I have data as Grid % as level wise and rating wise for IDC and BPO entities as below.
I am looking the out put as below:
Condition : Grid%
1) IDC, level 10, CSAPG rating : 15.65%
2) BPO, level 10, CSAPG rating : 21.00%
3) IDC to BPO move effective 01-Dec-15,level10,CSAPG rating : Sep-15 to Nov15: IDC grid% as 15.65% and Dec-15 to Feb-16: BPO grid as 21.00%

Kindly help me on this. Please let me know if you need more information on this.
IDC Entity grid% by level and rating:

8 18.65% 14.00% 9.00% 0.00% 9.00%
