Change Customer# to Entry Date and operate same as Appt date

I tried to change this sheet to make the changes I wanted but nothing worked for me.
Column named Appt date asks for 2 dates. A beginng date and ending date. then shows the results.
I need the Column named Customer# renamed to Entry Date and then ask for 2 dates. A beginng date and ending date. then shows the results.
in the results Customer# would be renamed to Entry Date. here is a screen shot. and the File. thanks
