Auto Updation of Data from User Form(VB) to Excel sheet

Dear All,

I need a help in auto updating the data from the user form(VBA) to Excel sheet.

For Example :
As soon as i enter the data in the User Form (VBA) and Submit it ,

then I should have my excel sheet being auto updated.

Attached is the User Form(Screenshot) and also the expected form in excel sheet.

Hi, Could you put the user form in your excel file so we dont have to rebuild it? Then it would be easier for someone to help you.
