Fetching row and column headers for a series of TRUE cells

There is a department in our firm that provides me with input for new projects in this format:

Based on the cells that include a "y", I compile a new worksheet and send it to another department for pricing:

I'd like to link these forms with formulas so that changes to the information on the input sheet will automatically be reflected on the other sheet without the execution of a macro.

Using Excel 2013.

Suggestions?

Appreciate your reply! How did you populate the first column? In the file you attached, I don't see any formulas in these cells.

What formula would allow us to populate this column automatically, selecting only those items with a "y" value in the original list?

I appreciate your responses, but they are focusing in the wrong direction. I am looking for help to use the information from the first table to populate Column A of the second table:

When the boss adds a "y" to the top table, I want it to create a new row in the bottom table:

Is there any way to do this without a macro?

A few questions to make sure I understand the formula:

Can you tell me a little more about the %% you included in the IF statement, I don't really understand what they are doing...Is that just a formula shorcut for dividing by 100 twice?

***Update - I have spent a long time studying this formula. Really elegant solution. Thanks again -- so much!
