cant get 3 slicers for a simple pivot table to work

I have a source data table with 900 employee records showing their employee group, their employee name, and then "yes" or "no" whether they have issue Type 1, issue Type 2, and/or issue Type 3.
I need to create a pivot table with
Column A=group,
Column B=count of Issue Type 1,
Column C=count of Issue Type 2, and
Column D=count of Issue Type 3.
The value in the data table for each of the 3 Issue columns is either Yes or No.

So I created a pivot table and then inserted a unique slicer for EACH of 3 Issue Types columns... but the pivot wont display the unique totals of Type 1, Type 2, and Type 3 issues. It displays only 1 group and then it displays the same Count of Issues for all 3 types.

If i delete the No value from the data table for the Issue Type columns and replace them with a blank, then the slicer will show the correct totals, but if I leave the No values in with the yes values then only 1 group is shown and it shows the same value for all 3 types.
really puzzling...

Does the attached workbook's pivot at cell H3 give you what you're looking for?
If so see the one step Power Query query feeding the pivot.

thanks, i have excel 2016, is Power Query an add-on for excel? I dont see it.
