Tax worksheet and in how do i export csv , how many fields ?


Herewith is Kenya Revenue PAYE (pay as you earn) worksheet for employees to deduct tax from salaries,
the trouble i am having is how many fields do i import PAYE csv , i deleted some columns and retain others, however, am not successful.
I tried by saving worksheet B-employee details as csv
A 9 digit PIN no may be used for your workings:-
ie. P051109164C to test data

Refer to worksheet B-employee details

If you can tell me how i can put in how many fields in csv file to import the same?
The output (answer) should be in csv file , of course.
