Simple "IF" Statement not working - help

Based on my "IF" formula, it should be returning the word "Exclude" but is not. In lieu, it is running the false statement. This should be super simple but not working. I am assuming that I am overlooking something.

Any help is appreciated. I tried to attach, but forum said the file was too large. Here are screen shots:
