Separate names with formatting from a column

I need some correction (below) in formula if possible.
please see attached file when I am applying this formula getting like this and need like below.

1. Remove all (JT)
2. Row 6 - If it is an individual person the middle initial seems to be there still - is there a way to remove?
3. Remove all (ETAL)
4. Row 47 - if there is no middle initial it is just merging the names - is there a ways to not do this?
5. Sometimes there is NO . Period after the middle initials, is there a way for these to still be removed?
