How to get Total in Userform Text Box based on some condition.

I have an excel file and user form having some textboxes.

Now what i want ?
I require total for a name according to the two condition.
First condition is that the total must be based on month
Second condition is that the total for "Tax Paid/Tax Unpaid"
And the most important is that the Cheque date must be the most recent .

You do needed modifications, which needs then use it with Your userform
... then You got it = It's possible, You can do it ... as You want it!
I do not use those forms at all.
