How to extract the data from the row text

I am attaching herewith a sample data in which the data has been truncated. Kindly suggest a formula to fit the data into respective places.

Solution is required as per sample file.

In fact, this data has been converted by using OCR from scanned PDF I have used the website avera. pdf. In some cases it works perfect. I have been Anyway, the employee name and code number will also serve the purpose. I have so many documents to type.
