Help with VBA code from a survey form

Hi!
I need a form that performs a search on the data in the worksheet. For that, I need to filter the data of the first and second columns through two comboboxes. The data from the other columns automatically appears in the textboxes. With just one comobobox I did it, but with two I couldn't (The second comobobox depends on the selection of the first). Can someone help me?! Here's an example of what I need. Thanks!
